Dental Office Letterhead Example
Willow Creek Dental · Logo left
Soft teal and a leaf mark feel calm and clean. Medical letterhead should reassure before it informs.
Pediatric Clinic Letterhead Example
Riverside Pediatrics · Color band
A friendlier blue band with a warm accent works for a pediatric practice without looking childish.
Physical Therapy Letterhead Example
Okafor Physical Therapy · Minimal
Clean lines and a green accent suggest health and movement, and they keep referral letters easy to read.
Word downloads have a simplified, editable header without the sample logo. Choose Customize with your logo to add your own logo or print the full layout.
What to include on medical letterhead
- Practice name and, if it's different, the legal entity name
- Provider names and credentials, such as Jane Smith, MD or Alex Lee, DDS. Group practices often list every provider, or just the medical director.
- Specialty in the tagline, for example Pediatrics, Family & Cosmetic Dentistry or Sports Physical Therapy
- Address, phone and fax. Fax is still widely used for referrals and records requests in US healthcare, so many practices keep it on the letterhead.
- Email and website, using a general practice address rather than a personal one
- NPI or license number (optional). Some practices add an NPI to letterhead used for prescriptions, prior authorizations or letters of medical necessity.
Where doctors and clinics use letterhead
Medical letterhead appears on referral letters, doctor's notes for work or school, letters of medical necessity for insurers, prior authorization requests, fitness-to-travel letters, patient recall and welcome letters, and records-release correspondence. Because these letters are often scanned, faxed or photocopied, plain and readable designs work better than delicate ones.
Design tips for healthcare letterhead
- Calm colors. Blues, teals and greens are common in healthcare because they read as clean and reassuring. See Willow Creek Dental and Okafor Physical Therapy above.
- High contrast. Keep contact details dark enough to survive a fax. Very light accent colors can disappear completely.
- Room for clinical detail. A compact header leaves more space for the letter itself, which matters for long referral letters.
- Friendly, not childish. Pediatric practices can use a brighter palette, as Riverside Pediatrics does, while keeping the type professional.
Privacy reminder
Letterhead itself contains no patient data, but the letters written on it usually do. Follow your practice's privacy policies (HIPAA in the US) when emailing, faxing or storing letters, and avoid using personal email addresses on medical letterhead.
